Service Description:	Provides the following support services for individuals living with a serious mental illness or mental health concern Intensive Case Management Program Provides flexible, individualized and ongoing support with case management Develops individualized support plans to assist with personal recovery goals Dual Diagnosis Program Provides specialized services to adults living with a developmental disability and a serious mental illness Support services include but are not limited to access to psychiatric, occupational therapy, behaviour therapy, and cultural support services Community Wellness Provides programs and motivational support for increased active leisure among vulnerable adults Activities may include but are not limited to walking groups, fitness, bowling and cooking Provides a monthly food bag consisting of mostly vegetables to program participants Services and Programs: Housing Subsidy Program

Eligibility - Population(s) Served:	Individuals 18 years of age and older with severe and persistent mental illness Dual Diagnosis Program: Individuals with a coexisting developmental disability and a mental illness Community Wellness Adults 18 years of age and older with physical and developmental handicaps Adults with mental illnesses or acquired brain injury Seniors in aged 55 and over in a low income category
