Service Description:	Support for individuals living with a life-limiting illness, their family members, and individuals who are grieving a loss * offers a full range of services, free of charge, by a team of professional staff and specially trained volunteers * services can be provided at the hospice or in-home and include: community hospice palliative care supports intake and assessment individual care plan development individual counselling group support wellness activities music therapy day program and social club information, education and referrals Volunteers are an integral part of the organization and opportunities are offered for anyone interested in supporting individuals and families in the community Services and Programs: Caregiver Support Child and Youth Support Day Program Grief and Bereavement Support Palliative Care Support Palliative Support and In-Home Hospice
