Service Description: Clinic for youth who:

  • have been diagnosed with a mental health disorder and are at high risk of harming themselves or others, or
  • are at risk of developing impairments due to a mental health diagnosis


TAY patients receive helps  to:

  • be involved in their own care
  • learn new skills and coping strategies
  • maintain healthy lifestyles
  • return to school, work and/or daily life
  • smoothly transition into adulthood

The clinic offers:

  • virtual care via telephone or video call platforms (e.g. OTN, Zoom)
  • medication education and management
  • psychosocial assessment
  • up to ten individual counselling sessions and/or group programming. Family members may also be invited to attend individual sessions
  • support for transitions to adult mental health care, addictions services, educational and/or employment opportunities
  • referral to community services
  • flexible hours, including evening and weekend appointments


Services are offered for up to six months

Application: Youth can request a referral to this program from their family physician or psychiatrist
Eligibility - Population(s) Served: Youth aged 16-24 years * live in the Central West Region * have one of the following: a major mental illness ; new and emerging mental health concern ; suicidal threats and or ideation ; concurrent substance use
