| Service Description: | Through a partnership with the Central West Self-Management Program (CWSMP), Osler provides support for people with chronic health conditions, including diabetes * workshops are group-based and non-clinical Peer-led group sessions include chronic disease self-management, diabetes self-management and chronic pain self-management. Each six-week session runs for two and a half hours, once a week Topics include:
