Service Description:

Provides comprehensive geriatric assessment services to seniors in their own home * patients may receive care from a team that may include a home visit from one or more of the following:

Nurse practitioner
Occupational therapist
Social worker
Pharmacist
Geriatrician

Recommendations may be provided to patients with the goal of improving their health and overall quality of life * patient's family doctor will be given a report and an appointment with a geriatrician will be arranged if needed
