Service Description:	Provides support to Indigenous youth as they transition from the child welfare system into adulthood Provides one-on-one support to create an individualized Action Plan to determine goals and supports required to achieve goals Offers programming on many topics to support goals and help build social networks Support services are available in the following areas: Cooking assistance Cultural education/information Educational assistance such as enrolling in high school, college/university, upgrading courses Employment Financial planning and banking Health care such as finding a doctor Housing (affordable) Legal support/advice Social activities

Eligibility - Population(s) Served:	First Nations, Inuit or Métis off-reserve youth 16-24 years of age currently in or preparing to exit the child welfare system
