Service Description:	Delivers medical transportation services and assistance in making medical appointment arrangements Provides transportation from/to Seine River Reserve to any medical appointments (doctor, dentist, optometrist and mental health counselling) in the town of Fort Frances Provides assistance with making arrangements for any medical appointments out of town such as booking appointments

Application:	Requires 24 hour advance booking; exceptions made for emergencies Must have medical trip authorization form signed by doctor/health professional during appointment and return it to Health Services Office Advance cancellation recommended
