Service Description:	Supports for individuals living with prediabetes or diabetes Provides education and services for the prevention, management and treatment of diabetes Incorporates both traditional and contemporary healthy living services focused on reducing the risk of developing Type 2 diabetes Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes Collaborates with other primary care team members to offer: Case management and screening services Dental screening Diabetes and preventative education Foot care and chiropody services Insulin adjustments Medical treatment Nutritional counselling Referrals to other health care professionals Regular bloodwork Retinal screening Traditional healing services Services are delivered through: Individual counselling Couples and family sessions Group sessions Community workshops
