| Service Description: | Supports individuals with intellectual disabilities of all ages and their families in accessing resources and opportunities in their community including: 
 The Community Support staff offer services in a variety of languages including: Cantonese; Mandarin; Hindi; Punjabi; Urdu and Tamil. | Application: | For intake to our Outreach Program, call and ask to connect with one of our Community Support Coordinators * Outreach Services: Any person who lives in Southern York Region and who has been assessed as having an intellectual disability may access the Community Support Program (documentation of assessment by a certified Psychologist may be requested) * referrals are accepted from family members, doctors, educational professionals and/or community service providers * In-Service: i.e. for group homes, day programs and Supported Independent Living - must register through the Developmental Services Ontario to be placed on regional priority list for services in Central East Region | ||
