Service Description: Community based GAIN clinic provides comprehensive geriatric assessments to address the complex needs of many older adults.  Assessments are done in-clinic or in-home, pending eligibility. The inter-professional teams work with older people and their families to develop care plans that support frail seniors with complex health issues so they can remain living at home. Staff include: 
  • Nurse Practitioner(s)

  • Pharmacist
  • Behavioural Support (BSO) Nurse
  • geriatrician support
  • administrative team members
Fees: No fees for clinical services
Application: Anyone may refer, but referrals are screened and triaged for appropriateness.
