| Eligibility - Population(s) Served: | Seniors living in the Etobicoke, Woodbridge, Malton and Brampton areas (Ontario Health atHome-Brampton (Central West) Seniors (65 yrs and up) with a mental health issue (dementia excluded). Adults over 55 yrs can also be seen if they have complex medical issues in addition to the mental health issue | ||
