Service Description: Hospice services available at no cost to clients, their loved ones and caregivers * services include:
  • end-of-life resident care
  • individual palliative support
  • in-home volunteer visiting
  • caregiver support
  • information, referral and health system navigation
  • Advance Care Planning
  • Grief and bereavement counselling and support groups

Application: Intake and Assessment done by staff * referral from primary care provider to residence admission
Eligibility - Population(s) Served: Any person who has a life-threatening or life-limiting illness and requires end of life support (no age restrictions) * any person, family or friend who is experiencing or has experienced loss and is in need of support
