| Service Description: | Ontario Health atHome provides in-home and information and referral services by care coordinators in the Lakeridge Health Corporation - Oshawa Hospital. The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home services upon discharge from hospital and to link patients to other useful community resources. In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language therapy, dietetic services and social work services. | ||
