| Service Description: | Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services * They also assist the client in planning for discharge when services are no longer required * Care coordinators are responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services * The services provided through Ontario Health atHome may include:
