| Service Description: | Outreach is a mobile program serving various high-risk neighbourhoods in the Sarnia area * using a community development model the program is designed to assist at-risk families using a "Hostess" approach which seeks to invite socially isolated families to participate in various Outreach sponsored activities and acts as a conduit to direct them to useful community supports * conducts a variety of activities in conjunction with other service partners to achieve its mission of early identification and intervention with childhood developmental and behavioral concerns | ||
