Service Description: The Atikokan Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Dietitian, Mental Health Worker, Pharmacist and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
Medical care team focused on family and community health
  • Offers timely access to high quality primary care
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Administers various health programs
  • Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM)
Provides health care services including but not limited to:
  • Chronic disease management
  • Diabetes program
  • Health promotion
  • Disease prevention
  • Nutritional counselling
  • Mental health counselling
  • Exercise groups
  • Ear flushes
  • Immunizations and injections
  • Medication review
  • Women's health
  • Wound care

Fees: Service - Some tests and/or forms are not covered by Ontario Health Insurance Plan (OHIP); Most services covered by OHIP

  • Call for appointment

  • No referral required for most services

  • Referrals from Physician/Nurse Practitioner required for Psychiatry services

  • Pharmaceutical counselling/medication checks available in a private setting; appointment required
