Service Description:	Provides education and services for the prevention, management, and treatment of diabetes Provides case management and screening services Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes Advises on developing and implementing an approach to care and management of diabetes Communicates patients' concerns to family physician Assists individuals in finding appropriate services Provides referrals to other health care professionals

Eligibility - Population(s) Served:	Individuals diagnosed with either type 1 or 2 diabetes, pre-diabetes, gestational diabetes or those who may be at risk of developing diabetes
