                        Primary health care focusing on health promotion and prevention to the residents of Bramalea * services include disease prevention, management and treatment, as well as rehabilitation and health promotion for all stages of life * primary healthcare teams offer:
 Interdisciplinary team of health professionals: physicians, nurse practitioners, support and care navigators, dietitian, diabetes nurse educator, physiotherapist, health promoter, medical office assistants, social worker/counsellor collaborate to provide an approach to health care to address health and social needs  |
