Service Description:	Offers permanent housing and housing support. The program is offered to women and men, 16 and over, who have a serious mental illness and are homeless or at imminent risk of homelessness. Permanent housing is offered through a rent supplement program. Case management is provided to assist clients to maintain housing, address recover goals and access community resources and services.

Application:	Referrals are accepted from all sources, including self-referrals. Referrals are made to SHIP's central intake department. Please call or complete the online application form.
