Post-operative escort and surgical transition support - dedicated one-on-one companion and Personal Support Worker (PSW) care for individuals undergoing day surgeries, outpatient procedures, or hospital discharges * includes: pre-surgery hospital accompaniment and waiting room care, safe transportation coordination, accompanied discharge home, post-operative settlement (medication reminders, meal prep, comfort checks), and initial at-home monitoring to ensure safe recovery Five free one-hour visits from five care companions trial 30-minute free consultation and free in-person assessment visit |
