Service Description:	Supports individuals with heart failure and/or COPD including chronic bronchitis and emphysema Provides equipment/kits necessary to measure vital signs and health information which are monitored daily by nurses who take action as necessary Participants are provided with equipment, training and weekly health coaching sessions

Application:	Referrals can be made by a primary care provider or a health care agency, but self-referrals are also accepted
