Service Description:	Enriched Supported Independent Living (ESIL) Offers support services in an individual's own home for those who do not require 24 hour support Supports vary with each person and can include budgeting, attending medical appointments, daily meal preparation, housekeeping and developing relationships Intensive Supported Independent Living (ISIL) Uses the "Housing First" model for individuals with significant barriers to housing Provides supports for individuals who have difficulty securing and maintaining housing Outreach Supported Independent Living (OSIL) Provides case management for individuals with complex needs including an intellectual disability, serious mental illness, and addiction Supports individuals with the transition from youth to adulthood

Application:	Initial application required through Developmental Services Ontario (DSO) (1-855-376-6673) Supports are available from 2-24 hours per day, 7 days per week

Eligibility - Population(s) Served:	Individuals 18 years of age and older with an intellectual/developmental disability
