Supports for people living with prediabetes or diabetes
Education Clinics: certified diabetes educators, including registered dieticians and nurses * work in cooperation with physicians, pharmacists and family members * individual and group appointments * referrals and telehealth sessions with specialists for advanced care Education on:
Outreach Sites: screening assessments to determine risk of developing diabetes * individual and group education and counselling Renfrew County Pediatric Diabetes Education Program: at Pembroke Regional Hospital or through Telehealth * in partnership with the Children’s Hospital of Eastern Ontario (CHEO) Services and Programs: |
