Service Description:	Patient-centred education and support to help people living with Diabetes or Prediabetes to manage their disease Services include: one-on-one sessions with registered dietitians and registered nurses (certified diabetes educators) ongoing information and education  insulin pump starts and support endocrinology clinics (referral to the Endocrinologist is required) group programming which includes activities and cooking

Application:	Referrals from Physicians, Nurse Practitioners, or self-referrals are accepted
Eligibility - Population(s) Served:	Adults 18 years and older with Type 1, Type 2 or Gestational Diabetes as well as those with Prediabetes
