Outreach workers support people to create a plan to identify goals which the individual self-identifies as being important * goals include ways to better manage chronic health and/or social determinants of health Outreach workers provide support and coaching as part of a team, together with staff and clinicians of Mount Forest Family Health Team, Minto-Mapleton Family Health Team, East Wellington Family Health Team, Upper Grand Family Health Team, Ontario Health atHome - Waterloo-Wellington, and Canadian Mental Health Association -Waterloo Wellington, and work in collaboration with many community partners to assist and support clients :
