Service Description:	This program provides case management services to people with serious and persistent mental illness. Clients are supported to live independently in the community through assistance with activities of daily living, advocacy, medication management and referrals to community resources. The program helps clients to manage the symptoms of their illness through a model of psychosocial rehabilitation.

Application:	Referrals are accepted from organizations and self-referrals by calling, emailing or by completing a referral application available on the website.
