Application:	Potential clients are identified by the hospital or via Ontario Health atHome - North West Area and bookings must be received a minimum of 24 hours in advance

Eligibility - Population(s) Served:	Individuals 55 years and older who: are able to direct own care or caregiver are medically stable are able to be left unattended are mobile, ambulatory (able to self-transfer), and not need the assistance of more than one person to manage stairs pose no threat to the health and safety of self or workers AND do not live in a long-term care home or Assisted Living where 24-hour care is provided
