Service Description:	Provides a multi-disciplinary team including representatives from social work, pharmacy, nursing, dietitian, family medicine, and the Alzheimer's Society to address memory issues Services provided include: Assessment of issues such as delirium and depression based on a clinical reasoning model Assessment of functional and cognitive impairment Considerations in individual's fitness to drive Interventions may include but are not limited to: Medication changes Referrals to programs and support Driving status considerations
