Service Description:	Transitional clinic for people living with atrial fibrillation Services include: timely access from ED or Primary Care Provider to the first clinic visit education about AFIB and its management access to necessary cardiac investigations comprehensive management of AFIB and other comorbidities timely access to cardiology, electorphsiology, or specialty care if needed seamless transition to community care providers with 24-48 hours of visit
