Service Description:

Provide primary health care, health promotion and a wide variety of community programs to the Stonegate-Parklawn Community * identify individual and community needs and respond by providing accessible and inclusive health services and health promotion programs

Clinical Health Services Health services include:

medical services - primary health services
chiropody services - foot care
dietitian services - nutrition education and counseling
asthma education - one-on-one sessions and group sessions on information to understand asthma or other breathing problems
diabetes education - offer nutrition education and counseling throughout the whole lifespan, from infant to older adult

Community Programs Provides primary health care services and health promotion and community programs in the Stonegate- Parklawn Community * providing accessible and inclusive health services and health promotion programs * the programs focus on a variety of community needs such as physical activity, healthy eating, socialization, support groups, mindfulness, chronic disease management and more

Family Programs Support the health of individuals, families and communities

Fitness Program Offer free activities to promote physical health and well being

Seniors Programs Variety of programs for seniors in the community

Women's Programs Designed to reach out to women of all ages in the community * offer women focused services and activities, and especially welcomes women who feel disadvantaged or isolated * provide individual counselling and advocacy, and facilitates group workshops and activities on topics of special interest for women
