Service Description:	After hospital discharge services include: A phone call from Pharmacist upon referral from Lake of the Woods District Hospital (LWDH) Assistance with identifying and resolving medication related problems that may occur during transition from hospital to home

Application:	If deemed necessary, arrangements made upon referral from Lake of the Woods District Hospital
Eligibility - Population(s) Served:	Individuals 65 years of age and older who have been discharged from the Lake of the Woods District Hospital and are taking 4 or more medications or at least 1 Institute for Safe Medication Practices (ISMP) medication
