Service Description:	Assists patients living with asthma or Chronic Obstructive Pulmonary Disease (COPD) to better manage their disease process Conducts spirometry assessments to determine lung function Provides education and support to help set goals for individual's care Offers smoking cessation counselling if needed Arranges follow-up visits as needed Provides referrals to other programs and services
