Provides members with access to the expert advice and care management provided by Geriatric Care Managers * service is being offered exclusively to Mosaic Members as part of the hourly home care options * under the guidance of a registered social worker they are able to assess your living environment, with strong focus on ensuring that the older individual and their family are managing as safely and as well as possible Deliver care management and coordination with the caregiver and/or when family cannot be there * working with the family; as well as the Client Services Liaison, personal support worker (PSW) Provide counselling to the family member and Mosaic Member of those transferring from home to retirement or long-term-care facility * management and guidance to assist with this change and to make sure the transition is a smooth one Attend the specialist/medical appointment when family cannot attend, to ensure that the right questions are asked or answered and to relay this back to the family Family counselling Deal with crisis management, being available to problem solve and address the pressing issues to include the whole care team including caregivers and provide a patient centered care approach to include Alzheimer's and dementia, Parkinson's, palliative care |
