Service Description:	Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members Provides transportation within the community for individuals accessing the Nursing Station Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual Provides assistance with completing prior approval applications for long distance medical appointments Offers assistance with arrangements for accommodation and travel details for individuals attending medical appointments at Sioux Lookout Meno Ya Win Health Centre

Application:	Must make arrangements for out-of-town travel at least 2 weeks prior to date of appointment/travel Requires written proof of medical appointment Must have Confirmation of Appointment (COA) stamped/signed and return to Health Centre after appointment Advance cancellation recommended
Eligibility - Population(s) Served:	Members of Cat Lake First Nation who require assistance with Non-insured Health Benefits (NIHB) arrangements and forms, and assistance with medical transportation due to age, illness or disability
