Service Description:
  • Links patient and family members through early referrals to community services for non-medical management from the point of diagnosis, throughout the continuum of the disease
  • Connects individuals to dementia education training sessions
  • Provides active follow-up and monitoring of patients and their family caregivers to assist in reducing the number of patient crises
  • Works to reduce caregiver burnout and premature long term care placement
  • First Link referral partners include:
    • Community health centres
    • Geriatric specialists
    • Health Service providers
    • Nurse practitioners
    • Pharmacies
    • Primary care practitioners
