Service Description: Supports for children, youth, adults, older adults, and seniors * Integrated approach to physical, mental health, and social determinants of health * Progams focus on improving health and wellbeing and promote independence and dignity * Services include case management, psychotherapeutic counseling, supportive housing, adult day programs, assisted living services, meals on wheels, transportation, respite care, caregiver supports, friendly visiting * System navigation support and referral to other services
 
Assisted Living Services 24-hour on-site personal care and support services for older adults and adults with disabilities
 
Community Group Exercise Classes for Older Adults Community group exercise classes for older adults and people with mobility issues * aims to improve strength and balance to prevent injury and falls
 
Crisis Outreach Service for Seniors (COSS) A partnership between Woodgreen, LOFT, Reconnect, South Riverdale Commnity Health Centre and Haven Toronto * trained professionals providing crisis assessment, intervention and support * 6-8 weeks of case management and counseling support to deal with crisis * advocacy, service navigation, harm reduction, health assessments and linkages to primary care through dedicated Nurse Practitioners and behaviour support through behaviour consultants
 
Financial Empowerment Services Education, counselling and support to low-income members of the community
 
Friendly Visiting and Medical Escort Program Regular volunteer visits to older adults and people with disabilities who live alone * one-on-one socialization * companionship
 
Meals on Wheels Hot or chilled western and Chinese meals delivered around 11:30am-1:15pm Mon-Fri * special diet types available * meals consist of protein, starch, soup, bread and margarine and dessert
 
Respite Care Temporary relief for caregivers of people with complex medical conditions
