Have you heard of case managers?
I remember the first time I was hospitalized for a week after surgery. While some of it remains a blur (thank you demerol), I do remember how much I relied on the nurses and others who made up my health care team. If the person you're caring for is hospitalized, ask to speak with a case manager from your local Community Care Access Centre. Case managers can play a pivotal role in your loved one's care, assist with the transition from hospital to home and connect individuals to community health and social services to aid in their recovery.
