Share The Care™ - Local History

How Share The Care™ Started in the South West Region
The Share The Care™ model of care was introduced to the South West region of Ontario as a community information campaign in the summer of 2009. It was funded by the South West Local Health Integration Network (SW LHIN) and also received in-kind support from the South West Community Care Access Centre (SW CCAC).

Paul Cavanagh, Director of the South West End-of-Life Care Network, chaired the committee of community partners making Share The Care™ possible throughout the region. Eugene Dufour served as campaign coordinator until the campaign came to a close in spring 2011. Dufour, who is clinically trained, is a bereavement specialist and compassion fatigue educator who continues to consult with Share The Care™ stations at their request.
